Summary of the project :
The project aims at building an integrated health system defined by minimum hospital assistance and increased alternative health care (such as home care and nursing, social and health territorial assistance, preventative health care interventions). This shift in the paradigm of health care provision will be based on the analysis of the social and health needs of locals, as well as on the assessment of the addressibility, access, efficiency and effectiveness of the various types of health care providers in the designated area. In order to have an overview on the existing services and the possible strategic approach, an analysis will be made of the present status of infrastructure and services provided to the population and of the necessary adaptations.
This analysis will be organised by a mixed Flemish-Rumanian team of experts and spread over three evaluation seminars of four days in Cluj (two seminars) and Bucharest (one seminar). The preparative stage will be concluded with a one-week visit to Flanders to familiarise with the Flemish model. This study visit will be oriented to health and social policy makers and social organisations. In the next stage an integrated model will be set up, based on these made experiences. In this model, the existent gaps in the health care providers’ chain will be filled in by building modulated home-based structures for social assistance, which will operate in close collaboration with the health care network. In order to fulfil these tasks, an information system will be developed to allow information sharing amongst the social and health care providers in the territory regarding individual social and health information. Finally, the model will be presented on two evaluation seminars, organised by both the Flemish and Rumanian partners and oriented to the national and regional health authorities.
The model will be discussed and evaluated on its practical implementation possibilities. In the set model, an important accent will be laid on the health promotion and health education program for the population in order to allow them to understand the model and its benefits. The model will be finally presented in a handbook on an international conference and presented to the national authorities, the press, social organisations and health providers. On this conference, emphasis will be laid on the possibility of dissemination of the model to other regions of Rumania.
